Step-Wise Procedure

• Visit Citizen Service Center F-6/1 Islamabad or nearest Model Police Stations Ramna, Aabpara,Shalimar,Kohsar,Secretariat,I-9,Margalla or Concerned Police Station .

Following documents required:

1. 1. In case of CNIC & ITP Driving License Missing
   * Copy of CNIC & Copy of Driving License.
   * No affidevit required.

2. 2. In case of other Missing and Lost
   * Copy of CNIC.
   * An affidevit.

3. 3. In case of missing following items report to concerned nearby Police Station
   * Property Documents.
   * Arm License.
   * Checque Book,Bank Payorder, Saving Certificate etc.
   Documents required
   * Copy of CNIC.
   * An affidevit.
