Step-Wise Procedure

• Visit Police Khidmat Markaz located at Sector F-6/1,H-11 Police Lines or the nearest Police Station.

Following documents are required:

1. In case of Missing / Loss of CNIC and Islamabad Driving License.
   * Copy of CNIC and Copy of Driving License.
   * No affidavit is required.
2. In case of Missing / Loss of other items.
   * Copy of CNIC.
   * An affidavit.
3. Consult concerned Police Station, in case of Missing / Loss of following items.
   * Property Documents.
   * Arms License.
   * Cheque Book, Bank Pay Order, Saving Certificate, etc.
4. In case of Missing / Loss of Vehicles Documents.
   Documents Required:
   * Copy of CNIC.
   * An affidavit.
   * Pictures of engine and chassis numbers on a plain page; black & white or color prints are acceptable.
   * Vehicle will be inspected by Vehicle Verification Officer.
5. Applicant must be present
